iPico / IELLIOS Loaner

iPico / IELLIOS systems are can be serviced ONLY only in the UK diagnostic and/or repair center. Service and/or repair by individual or company other than the iPico / IELLIOS manufacturer is strictly forbidden and will void any warranty that may be in place.

A loaner will be shipped using two day delivery service from the UK upon receipt and acceptance of the complete information requested in the form and after the credit card authorization as provided.

Repair/Loaner Terms and Conditions:

Please be sure to follow the instructions below exactly to avoid any delay with the shipment of a loaner system.

• The faulty iPico / IELLIOS must be shipped to the UK manufacturer within two (2) business days after you receive the loaner iPico / IELLIOS. A late fee of $500 per day will be charged to the customer that has not sent the faulty device
• The iPico / IELLIOS Fault Diagnosis Checklist (see below) must be filled in on the internet, printed and shipped in the same box with the faulty iPico / IELLIOS to the designated authorized diagnostic and/or repair center.
• Once this Return Goods Authorization form is received and accepted, your Customer Service representative will designate the appropriate authorized center to ship your unit for service.
• The faulty iPico / IELLIOS must be packaged in the original box prior to shipping it to the diagnostic and/or repair center.
• A deposit of $7,880 must be processed prior to shipping the loaner. The deposit of $7,880.00 is fully refundable provided that: a) the Customer ships the faulty iPico / IELLIOS within two business days of their receipt of the loaner iPico / IELLIOS (verified by the shipment tracking number) to the designated authorized diagnostic and/or repair center; b) the Customer ships the loaner iPico / IELLIOS to the UK Corporate office within two business days of their receipt of their repaired iPico / IELLIOS.
• Any Customer who fails ship the faulty iPico / IELLIOS to the designated authorized diagnostic and/or repair center within two business days after their receipt of the loaner iPico / IELLIOS from the UK Corporate office, will be charged $500.00 per day until this loaner is received and accepted at the UK Corporate office.
• Once the repaired unit is received by you, the loaner iPico / IELLIOS must be packaged in its original box and promptly shipped to the UK Corporate office.
• The loaner iPico / IELLIOS must be returned to the UK Corporate office within two (2) business days after you receive your repaired iPico / IELLIOS.
• Any Customer who fails to return the loaner iPico / IELLIOS within two business days after their receipt of their repaired iPico / IELLIOS to the UK Corporate office will be charged $500.00 per day from the time they received their repaired iPico / IELLIOS until this loaner is received by Corporate.

Authorization: I agree with the terms and conditions regarding the Loaner iPico / IELLIOS. I authorize Arasys Perfector LLC to charge my credit card the deposit amount of $7,880.00 prior to receiving a loaner iPico / IELLIOS or IELLIOS.
Warranty Repair: Please refer to the terms and conditions of your Purchase Order agreement for your New iPico / IELLIOS, Upgrade and Extended warranty coverage.
Minimum Diagnostic/Repair Charges: If the Customer’s iPico / IELLIOS is found to be fully functional and no fault is found by the designated authorized diagnostic and/or repair center, then I forfeit my $7,880 deposit. In addition I understand that my credit card will be charged and additional $380.00 for diagnostic services. Additional charges may apply for shipping, handling and insurance.
Late Return Penalty: I understand that my credit card will be charged $500.00 per day if I fail to comply with the time restraints in sending my faulty iPico / IELLIOS system to the UK Corporate Headquarters for repair or for not returning my loaner iPico / IELLIOS as stated and agreed above.

I agree with the terms and conditions regarding the iPico / IELLIOS Loaner. I allow Ion Magnum LTD to charge my credit card the refundable amount of $7,880 prior to receiving an iPico / IELLIOS loaner. If the iPico / IELLIOS machine is concluded to be fully functional and no fault is found by a Corporate Technician, then I forfeit my $7,880 deposit, in addition to being charged $380.00 for services and shipping (Erroneous Return Penalty). There will be no charge if my machine has a fault and the fault has occured within the first year of full warranty.. I agree that I am fully responsible for shipping (including fees) if my warranty (1 year) is no longer in effect. iPico / IELLIOS Fault Diagnosis Checklist:
Does the system turn on? (Detector problem) Did you follow troubleshooting procedure? Turn the system off and unplug it. IELLIOS do you get a blank screen or a white screen and nothing else comes on? Unplug and plug your system back on. Does the problem persist? If so your IELLIOS needs to go back to the UK for repair. IELLIOS: Did the IELLIOS logo come on? Write down the exact error message that your IELLIOS gave you. Did you follow the instructions of the message? Did the problem persist evern though you turned the system on and off several times? Please call us. We may be able to send you by e-mail new software with instructions of what to do to repair your IELLIOS.
